Welcome to the Department of Applied Physiology and Kinesiology's Graduate Program.  Listed below are guidelines and forms that will assist you while you are completing your Ph.D. (Doctorate in Health and Human Performance) degree.  Please feel free to contact our Graduate Coordinator, Dr. Chris Janelle, or Program Assistant, Ms. Alison Davis , at any time.



To ensure successful completion of all degree requirements and paperwork required for graduation, graduate students should complete the following steps (the forms below are in portable document format (.pdf), and require Adobe's free Acrobat Reader to view - click here to download).

(1)  Apply for Florida Residency Status immediately.  Students with out-of-state status are strongly encouraged to initiate the process to receive an in-state classification.  We are limited in the monies we have available for tuition waivers.  Failure to obtain residency by the following academic year will probably jeopardize your chances of obtaining future tuition waivers.  Copies of documentation regarding the application process must be placed in your file in 100 FLG.

(2)  Register for a minimum of 3 semester hours each semester (2 hours for summer term).  We encourage full-time student enrollment!  Assistantships also have minimum credit hour requirements per semester.

(3)  Complete and return the General Information Questionnaire to 100 FLG by the 3rd week of classes (please keep Alison updated of any changes regarding your assistantship).

(4) During 1st semester the advisor must request in writing to the Graduate School any previous applicable graduate hours transferred into program. (Up to 30 credit hours may be requested.) Submit a copy of request to Alison, FLG 100, for filing.

(5) Complete the photocopy of the Recommendation of Supervisory Committee form by the end of 2nd semester & return it to Alison, FLG 100, for typing on an original. A minimum of 4 faculty members must agree to serve, at least 2 from within department, must agree to serve.  One member must represent the minor area of study.  Two members (including chair) must have doctoral directive status.

(6) Complete the Graduate Degree Program Contract form with committee’s assistance. Return form with appropriate signatures to Alison, FLG 100, for filing no later than the beginning week of the 2nd semester in program.

(7) If research of UF employment requires contact with animals, patients, human blood, or other potentially infectious material, you must attend our departmental class on OSHA standards (obtain details from room 100 FLG).

(8) Contact committee chair for details and forms necessary for projects involving human subjects before beginning any research.  (See form in packet.)

(9) All incomplete grades must be made up by the third week of the term following receipt of the "I" grade.

(10) Satisfy residency requirement of 30 credit hours taken in residence at UF (beyond 1st 30 hours counted toward the doctoral degree.)

(11) Complete qualifying examinations (written and oral) after third semester of graduate work beyond bachelor’s degree and before end of course work.  Schedule your exam with committee members. Complete Announcement of Examination form no later than 3 days prior to exam & return it to Alison, FLG 100, for typing on original form.

(12) After passing the written qualifying exams and obtaining approval of dissertation topic, have Alison, FLG 100, complete an Admission to Candidacy form. Take the form to the oral exam for committee’s signatures. Return the form to Alison.

(13) Register for a minimum of two semesters (if full-time resident) or a calendar year (if part-time) between passing the qualifying exam and date of receiving degree.

(14) After admission to candidacy, register for HLP 7980. To do so, complete the contract form in FLG 100 and return it to Alison.

(15) During pre-registration or first week of classes of final term, have a graduation check completed by the Department to insure compliance with GPA requirements for graduation, as well as hours required in major and minor and other requirments.

(16) Be enrolled for a minimum of 3 semester hours (2 hours for summer) in HLP 7980 during your final term.

(17) Remove any remaining "I" grades from records.

(18) Pick up Guidelines for Preparing Theses and Dissertations in 168 Grinter.

(19) File Degree Application with Registrar’s Office, S222 Criser, the first week of  the final semester. (See graduate catalog or deadline brochure for deadline dates.)

(20) Plan for dissertation defense between the third and ninth week of classes during term of intended graduation. The exam may be oral or oral and written.

(21) Complete Announcement of Examination form for dissertation defense  (at least ten working days in advance of exam)  and return to Alison, FLG 100, for typing on original form and distribution.

(22) Submit dissertation (final form; not xeroxed; signature pages unsigned; after Department Chair’s review) to the Editorial Office five working days before the final exam and no later than the published deadline. Included with this submission is a letter of transmittal from the supervisory chair to the graduate dean indicating the committee chair has read the dissertation and that it is ready for review by the Graduate School. Also, submit two additional copies of the title page, binding/microfilm receipts, copy of Announcement of Examination, and all other doctoral forms (obtained from 168 Grinter).

(23) See Alison, FLG 100, to receive the Report on Thesis or Dissertation and/or Final Examination form (original copy) one day before defense. She will type in the information so that you may have it with you for signatures after successful defense of your dissertation.

(24) Once committee approves dissertation, and the committee’s suggested revisions are completed, have committee sign off on the Report on Thesis or Dissertation and/or Final Examination form.

(25) Return the Report on Thesis or Dissertation and/or Final Examination form, complete with committee members' signatures, to Alison, FLG 100, with a copy of corrected original dissertation (all committee’s suggested revisions have been completed) for review by Department Chair and College Dean at least one week prior to the Graduate School’s final submission deadline. Also, return to Alison, FLG 100, the departmental copy of  Profile of Position after Graduation and the Graduate Program Evaluation Form.

(26) Submit the corrected original dissertation, 5 copies of the unpaged abstract, one extra copy of the title page, the biographical sketch, the Graduate School’s copy of the Profile of Position form (available in 168 GRI), and the Report on Thesis... form to the Editorial Office (168 GRI).

(27) Submit one additional copy of dissertation to FLG 100.

(28) Submit abstract for CRHPERD. Complete other materials in the Orientation/Final Exam packet and return to FLG 100 one week after defense.

(29) Complete materials at Career Resource Center at Reitz Union.
