Monthly Report Requirement
Required Deliverables for ALL 21st CCLC programs
Monthly Reports must be emailed to the 21st CCLC Director of Evaluation (David Gardner; dngard@hhp.ufl.edu). There is no requirement for signature, though submission of the reports confirms that the data reported to the Florida Department of Education has been verified and validated as accurately reflecting the operation of the 21st CCLC programs for which they are submitted. Programs on reimbursement must BOTH email the form to the Director of Evaluation, and print the form and send it to the Florida Department of Education with reimbursement claims.
The Monthly Report is used by the Florida Department of Education to determine technical assistance needs for 21st CCLC programs based on average daily attendance and actual versus proposed operation. Programs are encouraged to contact their Technical Assistance and Training Educational Consultant if experiencing challenges in student recruitment and retention.
Although programs are not required to submit copies of daily attendance rosters for each site and each day of operation, programs should maintain this documentation to support submitted reports and to be able to provide this information to the Florida Department of Education if requested.
