Improvement Plans
Improvement Plans assist sub-grantees improve operations and performance by providing a process to document realistic strategies, activities and timelines to address concerns and/or areas in need of improvement identified in the Evaluation Site Visit Report.
How to Complete an Improvement Plan
- Take notes of all information shared by the Evaluation Unit during the Exit Meeting. Specifically, identify all strengths, Areas in Need of Improvement and ensure you are in agreement with all information discussed that will be identified on the final evaluation site visit report
- Don't wait to begin developing the Improvement Plan. It is the responsibility of the sub-grantee to initiate the Improvement Plan.
- Once you receive your report, if you feel there is a discrepancy, you should immediately notify Evaluation Unit
- Due date for Improvement Plans is 10 days after the final evaluation site visit report is received by the sub-grantee
- Provide necessary contact information and date(s) of evaluation visit
- Identify Areas in Need of Improvement as stated in the evaluation site visit report- do not summarize
- Identify all previously implemented strategies to address the Areas in Need of Improvement
- Identify all new strategies to address the Areas in Need of Improvement
- Identify specific dates (ie: 1/20/10) of when strategies will be implemented and expected to be completed.
- Utilize Comments section to provide further information related to the Areas in Need of Improvement
