Mid-Year & End-of-Year Report Requirements
The Florida Dept of Education requires all 21st CCLC Programs grantee to submit operational data twice a year, online. Each grantee will be expected to submit data by the following dates:
Mid-Year Data Collection: January 30, 2011
Mid-year data will be comprised of monthly reports, progress towards program objectives, technology survey, general site operation survey, and transportation survey from July 2010 through December 2010. ALL 21st CCLC programs MUST complete monthly reports using the most up-to-date Monthly Report Template, which represents the FLDOE's Minimum Performance Standards for all 21st CCLC programs. Monthly reports for each site and month as well as the 2010-11 Mid-Year Reporting Spreadsheet must be submitted to David Gardner at dngard@hhp.ufl.edu.
End-of-Year Data Collection: July 31, 2011
End-of-Year data will be comprised of monthly reports from January 2011 through June 2011, achievement of program objectives, overall operations, academic impacts, behavioral impacts, teacher surveys, student surveys, and parent surveys (2010-2011 Data Collection Packet). As with the mid-year report, ALL 21st CCLC programs MUST complete monthly reports using the most up-to-date Monthly Report Template, which represents the FLDOE's Minimum Performance Standards for all 21st CCLC programs.
Monthly reports for each site and month as well as the 2010-11 End-of-Year Reporting Spreadsheet must be submitted to David Gardner at dngard@hhp.ufl.edu.
For the teacher surveys, student surveys, and parent surveys, programs should follow the submission instructions indicated in the 2010-2011 Data Collection Packet and mail the surveys to the following address:
